What Is Kohler’s Disease?

Kohler’s disease is a temporary loss of blood supply to the navicular bone in the midfoot, a condition known as osteochondrosis. It was first described by Dr. Kohler in 1908, and is sometimes discussed alongside a related condition called Freiberg’s disease, occasionally referred to as Kohler’s II.

The condition is most common in young children, particularly boys, and usually affects only one foot. Although the X-ray findings can look dramatic, the long-term outlook is excellent.

Signs and Symptoms

Kohler’s disease is sometimes discovered as an incidental finding on an X-ray taken for another reason. In other cases, parents notice signs that something isn’t quite right with their child’s foot.

  • Pain along the inside of the midfoot or arch
  • Limping or a noticeably altered gait
  • Mild swelling or tenderness over the navicular bone
  • Reluctance to bear full weight on the affected foot

How Kohler’s Disease Is Diagnosed

Diagnosis is typically made through a thorough clinical exam and x-ray imaging, which may show fragmentation or flattening of the navicular bone. A bone biopsy is not necessary. Because Kohler’s disease has been linked to a condition called tarsal coalition, careful follow-up is an important part of the evaluation.
