What Is a Keller Bunionectomy?
A Keller bunionectomy, also called a Keller arthroplasty, is a surgical procedure designed to eliminate pain and restore motion in the great toe joint. It is typically reserved for cases of severe arthritis, certain bunion deformities, previous failed bunion surgeries, or diabetic ulcerations near the joint.

Who Is a Candidate?
Keller bunionectomy is most often recommended for patients who meet specific criteria. Common indications include:
- Adults typically over the age of 55 with limited high-impact athletic demands

- Symptoms that persist despite shoe changes, padding, or custom orthotics
- X-ray findings such as joint space narrowing, bone spurs, or joint deterioration

What Happens During the Procedure?
The surgery itself is straightforward. A small incision is made over the great toe joint, and a portion of bone is removed from the base of the proximal phalanx. This relieves jamming, increases motion, and reduces pain. The space fills in with soft tissue to create a flexible “false joint.”
In some cases, a temporary pin is placed across the toe to maintain alignment while healing. The surrounding soft tissues are reattached, the incision is closed, and you are fitted with a surgical shoe. Casting is not required, and limited walking is usually permitted right away.
Recovery and Results
Most patients wear the surgical shoe for two to three weeks. If a pin was placed, it is typically removed in the office around weeks three or four — no second surgery needed. From there, you can get the foot wet, increase weight-bearing, and begin gentle range-of-motion exercises.
By around six weeks, most people return to regular shoes and routine activities. Mild swelling may linger for several months and often improves with a compression sock or supportive custom orthotics for balanced weight distribution.
