What Is a Plantar Fibroma?

A plantar fibroma is a benign growth of fibrous tissue that develops deep within the plantar fascia, the thick band running along the bottom of the foot. Unlike plantar warts, which sit on the skin, these nodules form inside the arch and often cause aching pressure, pain when standing, or discomfort inside shoes.

Surgical excision is typically considered after non-surgical options, such as custom orthotics, padding, stretching, or injections, have failed to relieve symptoms.

Who Is a Candidate for Surgery?

You may benefit from surgical excision if you experience:

  • Persistent pain or tenderness in the arch of the foot
  • One or more firm nodules along the plantar fascia
  • Limited relief from orthotics, physical therapy, or injections
  • Difficulty walking, standing, or wearing normal shoes

The Surgical Procedure

The procedure involves a long, often curvilinear incision along the bottom of the foot, extending from the heel toward the ball. Your surgeon carefully dissects through the fatty tissue to expose the plantar fascia and the fibromas within it. For multiple or recurrent fibromas, a more complete removal of the fibrous band (Steindler plantar fascial stripping) is often performed to lower the chance of recurrence.

Once the fascia is removed, the incision is closed with sutures, and a small drain is often placed to prevent fluid buildup. The foot is then bandaged to protect it during healing.

Recovery and Post-Operative Care

Recovery is methodical, and patience pays off. Typical milestones include:

  • Non-weight bearing with crutches for at least three weeks
  • Drain removal three to five days after surgery
  • Stitch removal between two and three weeks
  • Gentle calf stretching and soft-soled walking once healed
  • Return to most daily activities around one month
  • Return to sports and full activity around three months

Custom orthotics are usually recommended afterward to support the arch, and in-office physical therapy and MLS laser therapy can be incorporated to ease post-surgical pain and accelerate tissue repair.

Possible Complications

As with any surgery, potential risks include infection, swelling, numbness, scarring, and recurrence. Walking on the foot before it is fully healed can also affect outcomes. Your surgeon will review your individual risk factors in detail before the procedure.
