What Are Plantar Fibromas?
A fibroma is a benign tumor made of fibrous tissue that can develop anywhere in the body. When these growths form on the plantar (bottom) surface of the foot, they are called plantar fibromas. Unlike plantar warts, which sit on the skin’s surface, fibromas grow deep within the plantar fascia, the thick fibrous band that runs from the heel to the ball of the foot.

Non-Surgical Treatment Options
Most patients begin with conservative care designed to relieve pressure and reduce irritation. Stretching, padding, anti-inflammatory care, and in-office physical therapy may also be recommended to help calm symptoms before considering surgery.
Surgical Removal of Plantar Fibromas
When non-surgical measures no longer provide adequate relief, surgical removal becomes a reasonable option. Solitary nodules can be addressed with wide excision, although recurrence rates are higher with this approach. Multiple fibromas typically call for a more extensive procedure known as a Steindler plantar fascial stripping, which removes the entire fibrous band to reduce the risk of return.
The procedure uses a curvilinear incision along the bottom of the foot, extending from the heel to the ball. The surgeon carefully separates the fascia from surrounding fat, nerves, and deeper tissue before closing the wound, often with a temporary drain in place.
Recovery and Post-Operative Care
- Non-weight bearing with crutches for at least three weeks
- Drain removal typically 3 to 5 days after surgery
- Stitches removed between 2 and 3 weeks post-op
- Gentle calf stretching and soft-soled shoe wear once healed
- Walking around for 1 month, full activity and sports for around 3 months
Functional custom orthotics are strongly recommended after surgery to support the arch, since removing the fascia naturally weakens it. Some residual tenderness near the incision is normal as the area continues to heal.
Possible Complications
As with any surgery, risks include infection, swelling, numbness on the bottom of the foot, and uncomfortable scarring, especially if the incision is stressed too early. Recurrence is also possible, although removing the full fascia significantly lowers this risk. Your surgeon will review all individual factors with you before proceeding.
