What Is a Giant Cell Tumor?

A giant cell tumor of the tendon sheath is a benign, noncancerous soft-tissue mass that arises directly from a tendon. These firm, nodular lumps most often appear on the toes, top of the foot, or sides of the foot, though they can also develop deep within the foot. They enlarge slowly and rarely exceed 4 cm.

Symptoms and Why It Hurts

Pain is the most common reason patients seek care. Discomfort typically stems from:

  • The mass presses firmly on the surrounding tissues
  • Interference with the function of the attached tendon
  • Pressure on the nearby bone, which can cause bone erosion over time

Because this bone erosion was once mistaken for cancerous invasion, giant cell tumors were historically thought to be malignant. We now know the erosion is due to pressure, not invasion.

How a Giant Cell Tumor Is Diagnosed

A definitive diagnosis is typically confirmed by a pathologist after the mass is removed. During your visit, our team reviews your history, examines the lump, and may order imaging. X-rays can reveal the shadow of the tumor and, in 10–20% of cases, show bone erosion. An MRI may be recommended to map the size and depth of the mass before surgery.

Treatment Options

The standard treatment is surgical excision. In some cases, a cortisone injection may be tried first to shrink the mass. Surgery is performed in an outpatient surgery center using local anesthesia with IV sedation or general anesthesia, depending on the tumor’s location. After careful dissection from surrounding tissues, the site is closed, and a compressive dressing — and sometimes a splint or below-the-knee cast — is applied.

Recovery and What to Expect

  • Sutures stay in place for 10 to 14 days
  • Keep the foot elevated above heart level and the bandage clean and dry
  • Crutches may be needed for a few days up to three weeks
  • Most patients need at least one week off work; light-duty roles may resume sooner

Possible complications include infection, swelling, delayed healing, tendon weakness, small patches of numbness in the skin, or recurrence — though most procedures are smooth and successful.
