Has your child been born with a tiny toe that crosses over its neighbor, or have you noticed your fifth toe slowly riding up onto the toe beside it? This common digital deformity often starts as a cosmetic concern but can progress into a painful, rigid problem that makes shoes uncomfortable and walking awkward. Understanding the condition early gives families and adults the best chance at a smooth correction and lasting relief.
At Sweeney Foot & Ankle Specialists, Dr. D. Sean Sweeney, DPM, FACFAS, and Dr. Christy Leahey, DPM, FACFAS, bring more than two decades of experience treating toe deformities for patients of every age. Voted Best Podiatrist in The Woodlands by Living Magazine every year since 2012, our practice combines compassionate care with advanced surgical expertise to deliver outcomes you can trust.
What Are Overlapping Toes?
An overlapping toe occurs when one toe sits on top of an adjacent toe rather than lying flat. The fifth toe is most commonly affected, though a central toe—usually the second—can also overlap. The exact cause is not fully understood, but fetal positioning in the womb and hereditary factors both appear to play a role.
Signs and Symptoms
- One toe is visibly resting on top of another
- Difficulty fitting comfortably into closed-toe shoes
- Calluses or skin irritation on the overlapped toe
- Stiffness is the deformity that becomes rigid with age
- Pain or pressure during walking or sports
Conservative Treatment Options
Early diagnosis offers the best chance for non-surgical correction. In infants, passive stretching and adhesive taping over 6 to 12 weeks can help reposition the toe, though recurrence is possible. Roomy, properly fitted shoes and padding can also relieve pressure for older children and adults whose deformity remains flexible.
Surgical Correction
As the deformity matures, the toe becomes fixed, and surgery is typically required for true correction. A skin plasty releases the tight skin contracture, and a tendon release, along with soft-tissue work at the base of the toe, restores proper alignment. In more severe cases, a small pin is placed through the tip of the toe to hold it straight for up to three weeks. Patients wear a post-operative shoe or removable cast during healing, and splinting may continue for another two to three weeks after pin removal.
Recovery and Results
Most patients return to normal activity within a few weeks, depending on the extent of correction. Limiting movement at the surgical site is essential for the best long-term outcome. Following recovery, patients enjoy straighter toes, improved shoe comfort, and renewed confidence.
