What Is a Tarsal Coalition?

A tarsal coalition is an abnormal connection between two of the tarsal bones in the back of the foot or arch. This connection — made of bone, cartilage, or fibrous tissue — limits or eliminates motion in the affected joint, which often leads to pain in the joint itself or in surrounding joints that are forced to overwork.

When a coalition forms outside a joint, it’s called a bar. A bar links two bones that don’t normally touch, restricting nearby motion and accelerating wear and tear that can cause early arthritis.

Common Causes

Most tarsal coalitions are inherited and passed down through generations. Less commonly, they develop from outside sources such as:

  • Arthritis
  • Infections
  • Trauma or injury
  • Abnormal bone growth

Symptoms to Watch For

Symptoms usually appear during the teenage or early adult years, though some coalitions never become symptomatic. Common signs include:

  • A deep, aching pain near the ankle or arch
  • Muscle spasms along the outside of the leg
  • A stiff foot with reduced flexibility
  • A noticeably flatter foot, especially when only one side is affected

Keep in mind that flat feet alone do not mean a coalition is present — many other conditions can flatten the arch.

How a Tarsal Coalition Is Diagnosed

Diagnosis typically begins with a careful review of symptoms and a hands-on exam. X-rays are usually obtained, and a CT scan or MRI often confirms the diagnosis and reveals the type, location, and severity of the coalition.

Treatment Options

Treatment depends on the patient’s age, symptom severity, and which joint is involved. Conservative care is typically the first step:

  • Custom orthotics to reduce painful motion
  • In-office physical therapy to strengthen and stabilize
  • Anti-inflammatory medication
  • Cortisone injections for targeted relief
  • MLS laser therapy to reduce pain and inflammation

If symptoms persist, surgery may be recommended. Procedures range from removing the coalition to restore motion to fusing affected joints to stop painful movement. Recovery often includes a period of non-weight bearing, casting, and follow-up physical therapy to rebuild strength and range of motion.
