Lingering ankle pain after a sprain, a stubborn click in the joint, or swelling that refuses to fade can quietly steal months of activity from your life. Minimally invasive arthroscopy gives surgeons a direct, magnified view inside the ankle and subtalar joints — so problems can be diagnosed and corrected through incisions smaller than a pencil eraser. Clear information on how the procedure works, who it helps, and what recovery looks like makes the path forward far less intimidating.
At Sweeney Foot & Ankle Specialists, Dr. D. Sean Sweeney, DPM, FACFAS, and Dr. Christy Leahey, DPM, FACFAS, bring more than 20 years of trusted surgical expertise to every patient in The Woodlands and Magnolia. Voted Best Podiatrist in The Woodlands by Living Magazine every year since 2012, the practice pairs advanced technique with the warm, patient-centered care families have come to rely on.
What Is Ankle and Subtalar Arthroscopy?
Arthroscopy is a minimally invasive surgical technique that uses a tiny camera-tipped lens — between 2.0 and 6.0 mm wide — to inspect and treat the inside of a joint. Through a few quarter-inch incisions, your surgeon can view magnified images on a monitor and use specialized instruments to repair damage. The ankle joint (where the tibia, fibula, and talus meet) and the subtalar joint (between the talus and heel bone) are both well-suited to this approach.
Benefits Over Traditional Open Surgery
- Smaller incisions, less scarring, and reduced soft-tissue trauma
- Magnified, high-detail visualization of cartilage and ligaments
- Precise removal of scar tissue, loose fragments, and damaged cartilage
- Continuous fluid irrigation that protects cartilage during surgery
- Generally faster healing than open arthrotomy
Who Is a Good Candidate?
Arthroscopy may be recommended for patients with persistent pain following an ankle sprain, suspected cartilage injuries (osteochondral lesions), loose bone or cartilage fragments, impingement, scar tissue, or certain fractures involving the joint surface. It can also assist with select fusion procedures. During your consultation, your provider will determine whether arthroscopy is appropriate or an open approach is safer — for example, when a lesion is in a hard-to-reach area or when significant joint instability is present.
What to Expect During the Procedure
Arthroscopy is typically an outpatient surgery performed under general anesthesia, spinal anesthesia, or IV sedation with a nerve block. Two to four small portals are placed around the ankle, or two to three for the subtalar joint. Your surgeon may smooth damaged cartilage, drill micro-channels to encourage healing, remove loose bodies, or release scar tissue — all through these tiny openings.
Recovery and Results
After surgery, the ankle is bandaged and often placed in a removable boot or splint. Most patients are non-weight-bearing for 7–14 days, then progress to weight-bearing as tolerated. If a larger cartilage lesion is treated, non-weight-bearing may extend for up to 4 weeks. Physical therapy — including our convenient in-office options and MLS laser therapy to reduce inflammation — can speed your return to full activity. Mild swelling and soreness for several weeks is normal, so follow your provider’s instructions closely.
